Elevator issues: falling through, getting killed by 173

I'll try to recreate this with video proof and seed string, but as of 1.3.6 I still fell from an elevator once which was supposed to lead me down to the maintenance zone. And on another seed I was about to go down an elevator to 939 when suddenly I get killed by 173 with no reason at all except I never saw him close by before then. Or is this a new event?
