I went down to SCP-012's containment chamber in one of my runs. I went to the chamber without using the console and played through normally. Upon clicking the monitor, I was greeted with an MAV.

Is it happens every time?

I only tried it once. I can go back and test it if you want.

Is it just the save your playing on, or has this happen on every save?

It's been addressed. Several issues have come up with compatibility on X64 Operating-Systems. There's a fix for it.
