I shall be posting a few sketches of how 049 could look, I'm not sure if it would be better if he was skinny (We kind of have enough lanky SCPs.) or very large and muscular (To be imposing, strong, but not enough to rip doors like 96.) Also unsure about his clothing, I keep looking at the Phantom from Phantom of the Opera for inspiration.

The only problem I see is that after being "operated" on by 049, following the lore, you would reanimate as 049-2, which is a big part of 049's story, I could only see him added to possibly "operate" on npc's that would then become a new kind of enemy as 049-2.

You missed this part: If the player is touched by 049 they would fall down and die, but perhaps instead of having the usual Game Over screen we would see a lil cinematic (in first person.) of 049 operating on them, the screen would then fade to black (Simulating the passage of time.) and be resurrected, at that point the Game Over screen would then pop up. And I was going to suggest the "zombies" but wasn't sure if people would like that.

I know, I just meant because you wouldn't technically be dieing, but I guess "Game Over" doesn't always have to mean death.

Perhaps the cinematic would continue, and the player would be shot down by MTF?

You first pick up a document explaining 1128's appearance. Then there is a flooded room, which you have to swim across. Once you get into the water after reading the document, you will disappear into an ocean and have to escape from the monster? Similar to 106, I guess.
I don't know, not one of my best ideas.

Wouldn't mind it being put it, could be a nice fast paced chase sequence.

Though, you would have to some how force the player to hear/see a description, since if it's a document and people know about it they wouldn't willingly pick it up.

Though, when you do look at a Document in game (not in your inventory yet.) it doesn't tell you what it is untill you pick it up, so maybe you could have the document have a rare spawn rate, and once it's in the inventory it will be marked as "read" and BAM, there you go.

Not a bad idea, but what maniac would leave around a document that they KNOW has harmful effects?

the room could be a testing chamber evacuated at the begining of the breach. They have descriptions of the being that the d-class read for testign before getting into liquids.This would imply giving the being a appearance, wich I think is not given in the article.
