I decided to return to the workshop to construct a new extra-atmospherical vehicle (that's a term right?), a quick review of the last rocket's flight path brought me to this conclusion.

Centrifugal forces rip the fuel canisters from your rocket, causing the electrical systems to fail, trapping the kerbals in there as they slowly die from suffocation/heat exhaustion/g-forces.

This was the last image of the Phallustrom as it ̶s̶t̶a̶r̶t̶e̶d̶ ̶s̶p̶i̶n̶n̶i̶n̶g̶ ̶o̶u̶t̶ ̶o̶f̶ ̶c̶o̶n̶t̶r̶o̶l̶ ̶ reached the upper atmosphere, it suffered a fatal meltdown when the fueltanks exploded upon initiating the last launch phase.

The parachute safety precautions failed.
